This form can be used for donations from a Donor Advised Fund or IRA Required Minimum Distributions being used as a Qualified Charitable Distribution. Kindly mail or deliver this completed form to the Community Foundation of Lorain County at 9080 Leavitt Road, Elyria, OH 44035 no later than 8:00 pm Thursday, September 18, 2025. All check requests to your institution of choice should be made NO LATER THAN Friday, September 5th.
